MODULE FIVE-A: INTRODUCTION TO INDIANA'S ONLINE BLOOD TESTER RECERTIFICATION SCHOOL

This module applies ONLY to individuals who have attended and successfully completed an Indiana State Poultry Association NPIP Blood Testing School Class.

CHECKLIST

1. To register for the Online Blood Tester Recertification School CLICK HERE.  You will complete an online form to register for the recertification.  As part of the registration, you will provide updated contact information (address including Indiana county, email and phone) and choose your payment method.  Recertification costs $25 and must be PAID, before taking the Recertification Exam.

2. Only individuals that have successfully taken an Indiana Blood Testing School Class at Purdue SIPAC (Dubois) or the main Purdue campus (West Lafayette) can take the online test.

  • The online course is designed for re-certification of those who have already attended a class at one of the Purdue locations.

  • If you have not attended a school in the past, contact our office at ispa@purdue.edu or 765-494- 8517 and ask to have your name added to the Blood Testing School notification list.

  • Two blood testing schools are typically held in April of each year. Check this website in late January for further information.

3. You should complete all five modules before taking the recertification exam.

  • There is a short quiz at the end of each module for your review.

  • Complete all course activities carefully; the information covered may be critical to the health of your flock.

  • Make sure you understand the material in each module before proceeding to the next module.

  • Allow adequate time (20-30 minutes) to complete each module.

  • The Glossary and Additional Resources sections are supplemental materials for further information.

4. After the ISPA has confirmed your eligibility to recertify online, by receiving and reviewing your Recertification Application, you will receive a confirmation email.

  • This email will contain a link to the Recertification Exam and a password that you will need to enter on the website to access the exam.

  • There is no time limit when taking the exam, however if you leave the exam webpage, it will not save your progress so you should give yourself enough time to complete the exam in one sitting.

  • When you have answered all the exam questions and have submitted it through the exam webpage, it is emailed to the ISPA for grading.

5. After successfully completing all of the modules and passing the final examination, you will:

  • Receive a new Official Blood Tester Identification Card

  • Be recognized as a certified NPIP blood-tester by the Indiana State Poultry Association

  • Receive a poultry information packet full of poultry based material and, if requested, a new book of VS Form 9-2's.

6. You will need to repeat this course EVERY THREE YEARS in order to maintain your certification.
